Only official passport type pictures are accepted.

The fee for processing your visa application is $180.
If you want an expedited visa the fee is $210.
Before you fill out the form, please read all of the instructions very carefully. The Chinese consulate's office is very
particular. Thank you.
This is a new 2 page visa application form. The older one page forms are no longer accepted.
Your passport must be valid for 6 months after your return date to the US, and must have a blank page for a
visa on the pages which say Visas (not the first 7 pages of the US passport or inside back cover). The Chinese
Consulate will paste your visa onto a blank visa page.
If your passport will expire less than 6 months from your date of return or there is no blank page for a
visa, you must get a new passport before applying for the Chinese visa.
It is each passenger’s responsibility to fill out the form (One per person) completely in black or blue ball point
pen. You must use the same pen to fill out and sign the whole application on the bottom of the form.
Any incomplete forms are unacceptable. Please do not use markers, gel or Pilot pens as well as pencils or pens of
colors other than black or blue as it may cause the rejection of your application by the Chinese Consulate.
The form is a Chinese Consulate’s official document. Any damaged or faxed forms are unacceptable.
Some of the information requested is self-explanatory. Here are some helpful instructions for filling out the visa
application.
1.1 Your name must be exactly the same as shown on your passport. Surname is your last name; Your Middle
name is next; Given name is your first name.
#1.7 For your date of birth, please note order, year first, month, then day. #1.8 Place of birth.
#1.9 Passport type is Regular. #1.12 Place passport is issued.
#1.14 If nothing else applies please check other and specify next to it. If you are retired, write Retired.
#2.1 please check "Tourism". We only accept tourist visa applications.
#2.2 please check "Single entry"
#2.3 please put in your tour departure date in order of year, month and day.
#2.4 please put in your length of your tour.
#2.5 please write two CITIES you visit (nothing else!). (Beijing, Shanghai etc. See your tour itinerary.) Please
do not write Hong Kong, Tibet or Yangtze River Cruise.
#2.6 please do not check anything unless you do want Rush processing and you are paying the additional visa fee
(extra $30 in addition to a current US$180 visa fee).
Section 4 please put in all the information, including the employer/school full street addresses and zip
codes. Please, no PO Boxes.

REMEMBER TO SIGN YOUR VISA APPLICATION FORM and ATTACH Your Passport Photo!!!
PLEASE DO NOT WRITE ANYTHING ELSE IN YOUR VISA APPLICATION!!!
You must attach one passport photo on the form. (You may use a stapler, a plastic tape or glue. Please do not
use a paper clip.) The photo must be a 2" x 2", full faced, white background photo that can be taken at your
local camera store. Please do not attach a computerized paper photo, a cut out, or a damaged photo. Any
photos which do not match the Chinese Consulates' requirements will cause the refusal of your visa.
After you fill out the form completely, please send it to Dr. Anastacia D. Samson with your valid passport to arrive
at least 75 days before departure by certified or express mail, FedEx or UPS, some form of trackable delivery. Do
not send by regular mail.
Champion Holidays is not responsible for the refusal of your visa by the Chinese Consulate New York.
Champion Holidays only accepts tourist visa application for travelers who book a tour.
Your passport with the visa pasted inside, travel insurance policy, final itinerary and the names, addresses and
contact information of all the hotels in China will be returned to you around 2 weeks before departure by FedEx.
Let Dr. Anastacia know if you want it sent to your work.
If you need a passport earlier than 2 weeks before departure, please inform me in advance. An additional cost of
$30 may be required in order to process an expedited service. Please allow us at least 10 business days to obtain
an expedited visa after we receive your passport. You can not apply for a visa 10 weeks or earlier before
departure.
